Reflect the soleus muscle to expose the deep muscles of the posterior compartment.
- Clean the calcaneal tendon and sever it 5 cm proximal to the calcaneus. Remove and discard the portion of the gastrocnemius between the severed calcaneal tendon and the cuts made in Step 2C. (G 6.67A;N 503;Gl 32.20)
- Separate the soleus muscle from the underlying structures and reflect the soleus muscle in the superior-medial direction (leave it attached at the soleal line).
- Clean the flexor retinaculum at the medial malleolus. (G 6.67A;N 505;Gl 31.18B)
- Sever the flexor retinaculum to expose the tibial artery and nerve, and the tendons of the medial malleolus. (G 6.69C;N 505;Gl 34.38)
- Identify the tendon of the flexor digitorum longus muscle. Trace the tendon proximally to identify the muscle belly. Identify and clean the deep fascia from the flexor digitorum longus muscle . (G 6.67C;N 505;Gl 32.24B)
- Identify the tendon of the tibialis posterior muscle. Trace the tendon proximally and identify and clean the belly of the tibialis posterior muscle.
- Identify the tendon of the flexor hallucis longus muscle. Trace the tendon proximally and identify and the clean the belly of the flexor hallucis longus muscle .
Important Relationships
- The tendon of the tibialis posterior muscle passes posterior to the tibia (medial malleolus).
- At the ankle, the tendon of the flexor digitorum longus muscle is positioned posterior-lateral to the tendon of the tibialis posterior muscle.
- At the ankle, the tendon of the flexor digitorum longus muscle is positioned anterior-medial to the tendon of the flexor hallucis longus muscle.
