(ON THE LEFT SIDE ONLY) Clean and identify the deep nerves, arteries and muscles of the hand. (G 2.77 and 2.78;N 452;Gl 28.44B) THIS IS THE "DEEP PALMAR" DISSECTION

  1. Remove the palmar aponeurosis and the palmaris brevis muscle. (G 2.71;N 453;Gl 28.42B) Locate and retain the recurrent branch of the median nerve. Do not worry about keeping the other superficial nerves and arteries of the palm intact.
  2. Identify the transverse carpal ligament.  (G 2.74B;N 453;Gl 28.39) Protect the ulnar nerve and artery (superficial to the ligament) and median nerve (deep to the ligament), and open the carpal tunnel by severing the transverse carpal ligament along the midline of the wrist. Use a scapel to cut the tendons of the flexor digitorum superficialis and profundus muscles, and the lumbrical muscles at the level of the metacarpal-phalangeal joints. Cut the FDP tendons at the level of the transverse carpal ligament and remove them (along with the FDS tendons) to expose the deep palmar muscles. The FDS tendons were previously severed at the wrist in Step 5B. (G 2.77;N 429;Gl 28.39)
  3. Identify and clean the transverse and oblique heads of the adductor pollicis and interossei muscles. Specifically identify the 1st dorsal interosseous muscle and trace the distal attachment of this muscle to the dorsal digital expansion. You do not need to identify any other specific interosseous muscle.
  4. Identify and clean the deep palmar arterial arch where it emerges from the proximal aspect of the adductor pollicis muscle. (G 2.77;N 453;Gl 28.44B) Attempt to identify the palmar metacarpal arteries and the deep branch of the ulnar artery.
  5. Identify and clean the deep branch of the ulnar nerve. You may remove the proximal portion of the hypothenar muscles to more fully expose the course of this nerve.
Important Relationships
  • The tendons of the flexor digitorum superficialis and flexor digitorum profundus muscles pass posterior (deep) to the transverse carpal ligament.
  • The median nerve passes deep (posterior) to the transverse carpal ligament.
  • The deep branch of the ulnar nerve passes directly medial to the hook of the hamate.
  • The deep branch of the ulnar nerve is positioned directly anterior to the palmar interossei muscles.
