Standing, sitting, prone, or supine
For women, a gown over the non-examined side with the examined area exposed or a halter top is recommended. For men, removal of the shirt is requested.
Posterior and lateral to the scapula.

May I place my fingers on your shoulder area? (or point to the demonstrated area)
With the patient relaxed, place your fingers or thumb on the flat area on the superior and lateral aspect of the scapula. Palpation of the spine of the scapula first, as done here, may help locate the acromion process.
VH - close up of the posteriolateral skeleton showing the scapula.
