Calcaneofibular Ligament

Sitting
Ask the patient to remove shoes and socks.
Lateral to the patient.
 
Please sit down. May I touch the side of your ankle? (or point to the demonstrated area).
Please relax while I move your foot.
Guide the patient to sit down. With the patient relaxed and with permission, place your fingers on the area just plantar to the center of the lateral malleolus and/or toward the posterior aspect of the lateral malleolus. Ask the patient to relax while you invert the subtalar joint while the ankle is slightly plantarflexed. Palpate the calcaneofibular ligament as it is stretched in this position. Note that the tendons of the peroneus longus and brevis cross the calcaneofibular ligament superficially and these tendons also are stretched with inversion, but are placed in a lax position with the ankle plantarflexed. The portion of the tendons that cross the ligament are not covered by the superior and inferior peroneal retinacula. The direction of the peroneal tendons is anterior and plantar while the direction of the calcaneofibular ligament is posterior and plantar, which helps in locating the ligament.
