Sitting, sidelying, lying supine or lying prone
Ask the patient to roll up long pants and remove shoes and socks.
Medial, anterior or posterior to the foot being examined.
Please sit down (or other position). May I touch the inside of your ankle? (or point to the area).
Please relax. (If needed, locate the tibialis posterior muscle for reference by asking the patient to bring the foot inward.)
Guide or ask the patient to sit down. With the patient relaxed and with permission, place your fingers on the posterior aspect of the medial ankle. This artery/pulse is posterior to the tendon of the flexor digitorum longus. This tendon is posterior to the prominent tibialis posterior tendon, which is just posterior to the medial malleolus.

Add Gastrocnemius, soleus , tibialis posterior, flexor hallucis longus, plantaris, flexor digitorum longus and brevis and their muscles and tendons and the long plantar ligament on one side
